Ok, so you have decided to attend the Philadelphia Ultimate camp, Congratulations!!
Now all you need to do is complete these two easy steps!!
Step #1: fill out the following forms...
- Camp Application
- Medical Authorization Form
- UPA Waiver - Please check off the box for "$5 Youth One-Time Event Fee." The camp will cover the cost of this fee.
(Our insurance is obtained through the Ultimate Players Association (UPA). Their waiver also has a section to become a member of the UPA. Joining the UPA is OPTIONAL, but we do need the waiver section completed. If you are a UPA member, put your UPA membership number. You do NOT need to fill in your social security number.)
Step #2: mail all the forms in with your deposit to....
